Registration Information

The following records are required for documentation before a student can receive full admission.

1. Student Admission Form

2. Official Transcript of Grades

3. Current Immunization Records

4. Social Security number - from card

5. Birth Certificate

6. Copy of national test scores such as ITBS or Stanford Achievement Test

7. Copy of any special testing program, if applicable

8. Eye, Ear, and Dental Screening

9. Letter of recommendation from head of school or counselor - Student Tracking Record may be used

10. Parent – Student - Teacher Conference

11. Completion of School Entrance Forms
